Rulers can be customized up to 999/999/999 monarch points I think this mod sounds like what you’re looking for. Would need a lot of work but should be entirely possible, at least from the design point of view. There's quite a few just searching for 'Country names' on the workshop.
Only custom borders would be new, but then again, EU IV does allow that in its custom nations creation and HoI4 uses the same engine. AI playing as custom nations will take the decision too. HoI3 had custom start where you could modify politics, techs, industry and armies. Adds convert provinces to primary culture and religion decision that can be enacted after starting the game. unlocks more governments to allow further customization. Changes the usual 200 points available for customization to a huge number (99999), removes distance and ideas/traditions cap. This idea behind this mod is to keep the game as close to vanilla as possible for people like me that don't want to stray too much from the standard game and feels having too many changes cumbersome. As the name of the mod suggests this is made for people who want to create their custom nation however they like.
